Become a Member

Becoming a member of NAMII is simple. Please review the Membership Tiers and  follow this three-step process:

  1. Download the Membership Request Form and complete the request form in its entirety. Return it via e-mail to namii.info@ncdmm.org with the e-mail subject line specifying “Membership Request Form.” Submitted Membership Request Forms will be reviewed by NAMII leadership to ensure compliance with our requirements.

 

  1. Upon approval by NAMII leadership of a Membership Request Form, NAMII will provide a formal NAMII Application Form. Complete the application form in its entirety. Submit the completed application form and payment in full for membership dues to NAMII at:

  1. Upon receipt of your application and membership dues payment, NAMII will provide a formal, signed NAMII Membership Agreement.
